Creating a Support Request

Where is it initiated?

Anyone can create a support request by clicking the “Support Request” button in the upper right of the main website. This will bring up a pop-up form where you can submit a support request.

Support Request Form

When the Support Request button on the main website is clicked the below form will pop-up that the user can fill out.

All fields are required on this form. The only thing that is optional is uploading an image.

 

  1. Manufacturer – Select the manufacturer of your home from the drop-down list.
  2. First Name – Enter the user’s first name.
  3. Last Name – Enter the user’s last name.
  4. E-mail – Enter the email to be used to contact the user about the support request. A confirmation will be emailed to them.
  5. Phone – Enter the phone number to be used to contact the user about the support request. The agent would be able to contact them directly via this phone number.
  6. Home Serial # – Enter the serial number that uniquely identifies the home.
  7. Close of Escrow Date – Enter the expected close of escrow date of the home.
  8. Home Type – Select the home type from the drop-down list: Available Home, New Home, or ADU.
  9. Priority – Select the priority level of your request: Low, Medium, or High.
  10. Park Name – Select the name of the park that the home is located in.
  11. Space # – After selecting the park name, then select the space number in the park where the home is located.
  12. Address – The address of the park is automatically entered once the park name is selected.
  13. City – The city of the park is automatically entered once the park name is selected.
  14. State – The state of the park is automatically entered once the park name is selected.
  15. Zip – The zip of the park is automatically entered once the park name is selected.
  16. Subject – Enter a brief few words describing the issue/request the user has.
  17. Description – Enter in a detailed description of the issue/request the user has.
  18. Upload Image Button – If you have any pictures to upload you can click the “Upload Images” button where you can browse for files on your computer and select them. A preview of the uploaded image will show below the button. You can selected multiple images for the support request.
Once the form is submitted, an email confirmation will be sent to the email address on the form and then the support request will show up in the Support Request List for an agent to review.
